Health Screening Checklist

When you first sit down for your appointment, your stylist will ask you to go over our Health Screening Checklist! If you answer “yes” to any of the questions, we will require to have your appointment rescheduled for a later date.

Client Health Screening Checklist

Have you had any of the following symptoms you are unable to attribute to another health condition?

  • Fever of 100.4 degrees or higher, or feeling feverish.

  • Chills.

  • A new cough.

  • Shortness of breath or difficulty breathing.

  • Sore throat.

  • Muscle aches.

  • Headache.

  • New loss of sense of smell or taste.

  • Been in close contact with someone who has tested positive for COVID-19 or experiencing symptoms associated with COVID-19.

  • Been tested for COVID-19 in the last week.

  • Pending a test result for COVID-19.

  • Have household members pending a test result for COVID-19.

  • Recently traveled by plane within the last two weeks.


If you can answer yes to any of the above listed, please let your stylist know right away!  Per our company policies in place to ensure we continue to uphold the highest safety standards for our Steller community, we may ask you to reschedule your appointment for a later date.
